Ambroxol: Mucolytic and Secretolytic Agent

Ambroxol (Lasolvan, Mucosolvan, Mucobrox) is a mucolytic and secretolytic agent widely used for respiratory conditions with excessive or viscous mucus. It is the pharmacologically active metabolite of bromhexine and is one of the most prescribed respiratory drugs globally.

Ambroxol also has local anaesthetic properties and anti-inflammatory effects, making it useful not only for chronic bronchitis and COPD but also for sore throat (lozenges). Available as syrup, tablets, effervescent tablets, inhalation solution, and throat lozenges.

Mechanism of Action

Ambroxol stimulates surfactant production by type II alveolar cells, reducing mucus viscosity. It also activates serous gland secretion, enhances mucociliary clearance, and has anti-inflammatory effects by inhibiting cytokine release and reducing free radical production. Its local anaesthetic effect is via sodium channel blockade.

Indications & Use

Acute and chronic bronchopulmonary diseases with impaired bronchial secretion: bronchitis, COPD, asthma, bronchiectasis. Throat lozenges for sore throat and pharyngitis. Also used to promote lung maturity in preterm infants (off-label).

Dosage

Adults: 30 mg three times daily (or 75 mg sustained-release once daily). Children (6–12 years): 15 mg two to three times daily; under 6 years: 7.5 mg two to three times daily. Adequate fluid intake is essential to enhance mucolytic effect.

Side Effects

Generally very well tolerated. Occasional: nausea, diarrhoea, oral numbness (lozenges), taste disturbance. Rare: hypersensitivity reactions, Stevens-Johnson syndrome (post-marketing). No significant drug accumulation.

Drug Interactions

Ambroxol has very few clinically relevant interactions. Combination with antibiotics (amoxicillin, erythromycin, cefuroxime) enhances antibiotic penetration into lung tissue — a therapeutically useful effect. Do not combine with cough suppressants (DXM, codeine) — the effects are antagonistic.

Contraindications

Hypersensitivity to ambroxol or bromhexine. Caution in first trimester of pregnancy, severe renal impairment, and rare conditions causing accumulation. Do not use with antitussive agents simultaneously.
